INTRODUCTION AND INSTRUCTIONS TO BIDDERS

INTRODUCTION

The City of Statesville is seeking proposals from qualified contractors for intersection improvements located at West Water St. and North Meeting St.

INSTRUCTIONS

1. Each offeror must thoroughly examine the Request for Proposal and contract documents to ensure that all requirements can be met. Proposals shall be submitted on the forms included within the ITB documents. See “Proposal Submittal Forms 1” for detailed instructions on what information to submit. The bidder shall sign his or her proposal correctly; proposals may be rejected if they show any omissions, alterations of form, additions not called for, conditional bid or any irregularities of any kind. The contract will be awarded to the lowest, responsive, responsible bidder. The City of Statesville reserves the right to reject any and/or all proposals.

2. Proposals and all supporting documents required to be attached can be emailed to Mark Taylor at [email protected] or you may mail your completed ITB to:

3. Proposals will be received only from bidders licensed under the North Carolina “Act to Regulate the Practice of General Contracting”. Contractors and subcontractors, in order to perform public work in the State of NC, are required to hold a State of NC contractor’s license(s) of the class required to perform the specified work.

4. See sample Service Contract for limits on Insurance requirements for work. The awarded vendor shall provide the Purchasing Agent an original Certificate of Insurance indicating that the vendor has in force the required coverage prior to the start of any work and agrees to maintain such insurance until the completion of the Contract. All insurance policies shall be with insurers with an acceptable rating, registered and licensed to do business in the State of North Carolina. Each policy shall provide a thirty (30) day notification clause in the event of cancellation, non-renewal or adverse change. In the event the insurance coverage expires prior to the completion of the project, a renewal certificate shall be on file with the City of Statesville Purchasing Agent at least fifteen (15) days prior to the expiration date. Failure to maintain the proper insurance will be grounds for termination of contract. 

5. Sales tax should not be included in any proposals and will not be included on City Purchase Orders. Sales tax should be included on and will be paid from vendor invoices. Proposals shall not include any Federal Excise tax.

6. By submitting a proposal, vendor, contractors, and/or subcontractors affirm they have read and accept our Purchasing Terms and Conditions. All vendors must READ and ACCEPT our Terms and Conditions by checking the box on the vendor application, the box is located at the TOP of the vendor application below Vendor Name. The vendor application and W9 must be submitted along with this proposal.
